VO2 Max: The Number That Predicts How Long You'll Live

Cholesterol gets the headlines. Blood pressure gets the prescriptions. But the number that predicts how long you will live is not on your standard bloodwork at all.

It is called VO2 max. And most people in Centennial have never had it measured.

What VO2 max actually is

VO2 max is the maximum amount of oxygen your body can use per minute, per kilogram of bodyweight. In plain terms: the size of your engine.

The bigger the engine, the longer you live. The better you function. The more life you absorb at every age without breaking down.

Why the number matters more than almost anything else

A 2018 JAMA study followed 122,000 adults. People in the lowest VO2 max bracket carried roughly five times the all-cause mortality risk of those in the elite range.

That gap is larger than smoking. Larger than diabetes.

One useful way to think about it is an aerobic bank account. You spend it slowly across your life. The bigger your starting balance, the more decades of independent, vigorous living you get before the account runs low.

And the biggest return in the study came from the first step: moving out of the bottom bracket into the next one was worth about as much as the difference between a smoker and a non-smoker.

Read that again. Not the top percentile. Just off the floor. That is the biggest return available to you in all of health, and it is almost entirely trainable.

The two-part protocol that actually raises it

There is no mystery here. Every elite endurance program in the world uses the same recipe.

Step one: build the base

Three to four Zone 2 sessions per week, 45 to 60 minutes, easy. Zone 2 means conversational pace. You should be able to talk in full sentences without gasping. If you cannot, you are going too hard.

This is the boring part. It is also the part that works.

Step two: find the ceiling

One interval session per week. The Norwegian 4x4: warm up ten minutes, then four minutes hard at a 9 out of 10 effort, four minutes easy, repeated four to six rounds. Cool down five minutes.

The 4x4 is the protocol with the strongest evidence behind it for raising the VO2 ceiling specifically.

Base builds the engine. Intervals raise the ceiling. You need both.

Training at altitude changes the math

Centennial sits at roughly 5,500 feet. There is meaningfully less oxygen available per breath here than at sea level, which means your Zone 2 pace is slower than a sea-level athlete's at the same heart rate.

This trips people up constantly. They chase a pace they read in a book, drift out of Zone 2 without realizing it, and spend months training in the gray zone that builds neither base nor ceiling.

Train by heart rate and breathing, not by pace. Especially here.

How to get your number

You cannot manage what you have not measured. A sub-maximal VO2 assessment takes under an hour and gives you a real number plus your percentile for your age and sex.
